To all Guard and Reserve service members and their families

Recognize your supervisor(s) for supporting National Guard and Reserve members by nominating them for a Patriot Award! They will receive a beautiful Department of Defense Certificate of Appreciation (framed if desired) and lapel pin. All members of the National Guard and Reserve and their spouses are eligible to nominate their employers!

Authority: 38 U.S.C. Chapter 43, Employment and Reemployment Rights of Members of the Uniformed Services (USERRA); 5 U.S.C. 571-583, Administrative Dispute Resolution Act (ADRA); 5 U.S.C. 552a, the Privacy Act of 1974; DoD Instruction 1205.22, Employer Support of the Guard and Reserve; DoD Instruction 1205.12, Civilian Employment and Reemployment Rights of Applicants for, and Service Members and Former Service Members of the Uniformed Services.

Purpose: To allow members of the National Guard and Military Reserve Component personnel and their family members to submit nominations for Employer Awards; and to allow civilian Employers to request a Statement of Support.

Routine Use(s): To process awards for Civilian Employer support of National Guard and US Military Reserve personnel; and To coordinate signing of Statements of Support with employers of National Guard and Reserve Component personnel; and To collect and report statistical data for annual reports to Congress and appropriate DoD agencies about Employer Support of the Guard and Reserve Activities.

Disclosure: Voluntary; however, failure to provide accurate contact information and other solicited information may delay or prevent ESGR from processing the request.

Instructions

1. Patriot Awards are awarded to individual supervisors, not to an entire staff or the employing organization as a whole.  Certificates are printed with your supervisor's name, so please enter information exactly as you would like it printed; you may add up to 5 names per nomination.

2. You may request that a Patriot Award be sent either to your home address or to the address of the ESGR State Committee in your supervisor’s state.

3. Complete the form below and submit. If you have any questions, please contact ESGR Outreach team at: osd.ESGR-VOL@mail.mil.

4. The average turnaround time for the Patriot Award to reach the address provided is 45 days from the date of submission. If you have not been contacted or received the Patriot Award 45 days after submitting your nomination, please contact the appropriate State Committee to get an update.
